lpr prints the specific page just fine

I suspect it is probably epdfview... I've seen similar things, although I guess it could also be ghostscript. Try printing it from the command line with lpr and see what it does. It's okay, I found it. I was asking about this setting in shell-fm: "P = enable/disable RTP". RTP stands for "record tracks to profile".
