USB Flash Drive Boot Failure

I've gotten it as far as to create some files on the flash drive.

How I installed puppy to drive:
Used that one HP thingy to format drive as fat16.
Downloaded puppy-1.0.4-mozilla.iso
Extracted image.gz, usr_gram.fs, and vmlinux to drive.
Downloaded syslinux.zip, and extracted that to drive.
Downloaded syslinux-3.11.tar.bz2, and used syslinux.exe to create boot sector(I think that's what the program does) because syslinux.com didn't work because of some problems writing to the flash drive boot sector.

Then I tried to boot up and I get a bunch of errors.

A bunch of "cp-full- cannot create" entries.
Followed by, "Mount: Mounting none on /usr Failed: Invalid argument"
"Pausing for 60 seconds"
After the pause it displays, "Exec: /root/.etc/rc.d/rc.local0: No Such File or Directory"

Then it ask me for login/password.

Attached is a image of the files on the drive after I rebooted. Any clues to what's wrong?

pup100 should be created but it's not there - is the drive full?

The drive still has an extra 438megs free. The files that were added during my boot attempt are only 280kb. Need anymore information?

Your picture looks like what I saw when I first got started. Read those two threads and see if that helps.
