Have a look at the files in "/usr/share/applications"
or "/usr/local/share/applications/"

For editing the structure have a look in "/etc/xdg/menus" and "/etc/xdg/templates"

Desktop Menu Specification

if you are using JWM or IceWM or maybe a few other window managers I have not fought of, the command to rebuild the menus is "fixmenus" then restart the window manager. In Desktop Environments like LXDE, XFCE, Gnome, KDE ,ect they refresh differently.
