I still would like to see one with white background, black lettering, so it could be printed on colored paper for those that do not have color printers, i.e laser jet such as mine.

Have you checked out my mini how to on making a CD insert?
It has two examples and can be found at this location.

An easier way would be to use SVG graphics in Inkscape
as SVG has very good scaling capabilities and thus templates
could be made for the different cases available.
Maybe Gimp also supports SVG but never used Gimp.
Once the wanted picture/s have been added to the "template",
it's a simple matter to export it to pdf (maintaining scale) and
either printing it home/office or emailing it to a print shop.
Most print shops are capable of "duplex" printing, meaning that
they can print on both sides of the paper/card so it's a simple matter
of supplying them the front and back artwork.
BTW, PostScript also supports scaling so the "template" could also be in
that format, just keep in mind that PS uses 72 DPI as a standard whilst
SVG uses 90 DPI.

You should be able to view it directly in FireFox, else download it and use Inkscape.
